2008 Charge Conference Forms


Note: You will need Adobe Reader installed on your computer before beginning. You can download it by clicking the Adobe Reader icon below.



The Charge Conference forms are being handled in a different manner this year. We are trying to use more technology to better equip the laity and clergy as well as save money. We encourage each charge/church to send the forms electronically.

Knowing that this is the first year to send the forms electronically and that some people may have trouble using it, you can also send the forms through the mail. Two different options have been provided for your convenience. If you encounter any problems, please use the form provided at the bottom of the page to contact the Webmaster.

Please follow the directions below when filling out the charge conference forms:


Option 1


1. Download and print off the Charge Conference forms by clicking the link below:

2. Save the PDF file to your computer (desktop, My Documents, etc.) and name it corresponding to your charge (i.e. PleasantGroveUMC.pdf, Bono_FisherSt.pdf, etc.)

3. Gather the necessary information from your charge and fill out the forms using Adobe Reader. Please obtain the necessary signatures on pages 7-9 (on the document) and mail them to the district office. (Note: The other signatures can be digitally signed.)

4. Fill out each text field where appropriate and save the entire document to your computer (i.e. desktop, My Documents, etc.)

5. Upload the completed file using the link below. Browse to find the completed file on your computer and hit upload. A confirmation page should appear.

Option 2


1. Download and print off the Charge Conference forms by clicking the link below:

2. Gather the necessary information from your charge and fill the forms out by hand or use Adobe Acrobat Reader. (Note: If you use Adobe Reader, please be sure pages 6-8 are signed.)

3. Mail the forms to the district office.
